Mark or Grade Amendment: Form M3
This form should be used to amend a mark and / or grade that has been previously reported at an Exam Committee.
Once submitted, the form will go to Programme Support team within Registry who will then direct it to the appropriate Exam Committee Chair for final approval.

Exchange Credit Transfer: Form M8
This form should be used to record the results on a student's return from exchange. Please ensure that you complete all of the necessary sections.

This form should be used to request compensation for a missing module. Please ensure that you complete all of the necessary sections.

Statement of Credit: Form M103
This form should be used to record and submit prior credit for a student. There are two M103 forms available, please review the guidance below to determine which is the correct form to use. Please ensure that you complete all of the necessary sections as per the Credit Entry Guidance.
For use when:
- students are transferring from another institution as part of a progression agreement and / or
- where generic credit for a whole stage of a programme can be awarded, and
- exemptions can be applied for that stage e.g. all Level 4 compulsories are to be exempted for students who will enter at the start of the Level 5 stage.
These forms can be authorised by the Admissions Team.
For use when:
- students are transferring from another institution and are entering part way through a stage of a programme,
- students who studied at Oxford Brookes previously are returning and have specific modules and marks from their previous study which can be used as credit and as exemptions from compulsory requirements,

Forms that students use to request changes (in circumstance)
The Student Request Forms webpage is where students go to request changes in circumstance, for example if they need to change mode of study or request a period of approved temporary withdrawal. Student Central will always advise that students consult their Student Support Co-ordinator or Academic Adviser prior to submitting requests of this nature.
