Mark or Grade Amendment Form F3
This form should be used to amend a previously reported mark and / or grade. This form should not be used to record Examination Committee moderations. These moderations should be entered within the moderated column(s) of the marksheet and signed by the Examination Committee Chair. Please ensure that you complete all of the necessary sections.

Statement of Credit Form F103

Deletion of Module from a Student's Programme of Study after the Deadline Form F99D
This form should be used to request the removal of a module from a student's programme of study after the normal deletion deadline, e.g Week 4 for postgraduate courses of study. You will need to confirm that the student:
- has not attended after Week 4 of the current academic session, and
- has not attempted any assessments after Week 4 of the current academic session.

Student Update Request Form (UK Collaborative Partnership Students)
This form should be used to request a change to the registration of a student studying on a course run at one of our UK collaborative partners. Please ensure that you complete all of the necessary sections. Students studying on courses run at Oxford Brookes should submit their own requests via the Student Request Forms page.

ACP and UK Collaborative Partnerships Chair's Action Form
This form should be used to amend a previously reported mark and / or grade for students studying at an ACP Partner or UK Collaborative Partner. This form should not be used to record Examination Committee moderations. This form will only be processed if authorised by the relevant Examination Committee Chair. The quickest and most efficient way for this to happen is for the Examination Committee Chair to submit the form themselves. If the form is submitted by any other colleague, there is likely to be a delay in processing the amendment while the authorisation is sought.

The Student Request Forms webpage is where students go to request changes in circumstance, for example if they need to change mode of study or request a period of approved temporary withdrawal.Student Central will always advise that students consult their Student Support Co-ordinator or Academic Adviser prior to submitting requests of this nature.
Request to Add an Independent Study Module Form F88
Use this form to add an independent study module to a student's programme. This will need to be completed after agreement on the specification of the study that the student is going to undertake.
